FLOATING CRANE BOOM DAMAGE INVESTIGATION – Indonesia

We were appointed to investigate a floating crane incident in which the crane boom collapsed onto a barge, resulting in significant denting to the boom’s mid-section. Our inspection focused on identifying the cause and evaluating the condition of all lifting components. During the investigation, we confirmed that the boom hoist wire was in good condition, having been recently replaced. However, we found that the luffing wire had completely disengaged from the drum, indicating that it had not been properly secured with clamps during the recent wire installation. This improper securing allowed the wire to slip free under load, leading to the boom collapse.
To ensure a comprehensive assessment, we recommended our client to send the hydraulic motor and gearbox to the manufacturer for further evaluation, to determine if any secondary failures contributed to the incident. This case highlights our capability in identifying mechanical, procedural, and operational contributing factors in crane-related incidents and providing clear recommendations for correct action.

HOW WE SOLVE THIS PROJECT

We conducted a detailed investigation into a floating crane incident where the boom collapsed onto a barge. Our assessment focused on identifying mechanical and procedural failures to ensure the root cause was fully understood and addressed.

We support clients in resolving equipment failures by delivering clear diagnostics, technical insights, and practical recommendations for safe crane operations.

Mechanical Inspection

We checked all lifting components and confirmed the boom hoist wire was in good condition.

Root Cause Analysis

We found the luffing wire had slipped from the drum due to improper securing, causing the collapse.

Follow-Up Actions

We recommended sending the hydraulic motor and gearbox for fur
